The eyelids are divided into anterior, middle and posterior lamellae.  They contain pre and retro septal fats. Orbicularis oculi, levator and Mullers muscles are closely involved.  Tarsus and septum act as curtain rail and curtain respectively with the medial and lateral canthal ligaments playing a role in the stabilisation of the lids. The lacrimal glands and naso lacrimal apparatus are closely related anatomical structures.

Preoperative evaluation should include a thorough medical and ophthalmic history along with a vision examination.  Base line tear production, intrinsic lid tone, lower eyelid support using snap back test and presence of malar support are important preoperative assessments. Symptoms of pre-existing dry eye should be evaluated preoperatively as they directly correlate with postoperative complication.

Lateral brow ptosis, hooding of the upper eyelid, ill defined supra tarsal fold, bulky lacrimal gland and medial fat herniation are the commonest upper lid cosmetic problems.  Fat herniation of a different degree with tear trough and malar palbebral groove, excess skin and loss of canthal definition are the lower eyelid problems. Transconjunctival fat repositioning with erbium laser resurfacing are preferred procedures in younger patients particularly of familial fat herniation.  Chemical deinnervation using Botox. fillers particularly autologous fat,  to correct tear trough and augment malar mound are adjunctive procedures.

In the upper eyelid the common abnormality is loss of the supratarsal crease with folding of excess skin, droopiness of the lateral brow and, to some degree, extension of the anterior fatty component.  There can also be medial fat herniation.  In the oriental eyelid the main abnormality is the droopiness of the anterior lamellar fat because of the anatomical variation.

In the lower eyelid, however, the sausage-shaped eyelid bags cause malar palperbral groove and tear trough deformities.  There will be loss of canthal definition due to ligamentous laxity.

We carry out a standard open sky upper blepharoplasty and we always carry out a unique suturing technique where the inferior part of the circumferential orbicularis oculi muscle is anchored to the levator aponeurosis.  This will recreate a well defined supratarsal crease.  In patients who need it we will also carry out a lateral brow lift which again is our technical exercise.  The medial fat herniation is cauterized simultaneously.

In the lower lid we carry out a skin flap technique with septal reset as opposed to excision of the fat.  We firmly believe that excision of the fat will cause hollowing of the lower eyelid area which would later give a sunken appearance.  We simultaneously carry out orbital muscle plication in order to strengthen the middle lamellar of the lower lid.

The aim here is to improve the contour without losing the volume, minimize the skin excision and maximize anterior and middle lamellar strength.

Here at the Wentworth Clinic we usually provide this facility more effectively under a local anaesthetic with oral sedation.

The incision is made along the natural crease in front of the ear or along the contour of the ear which will extend in a zigzag fashion just above the sideburn and which may also extend into the temple.  The skin flaps are raised, suspension sutures are placed with slowly resolvable suture material which will be anchored to deep structures in the temple.  After careful haemostasis the excess skin will be pulled in a vertical direction and the skin excess will be trimmed.  Simultaneously we will carry out a lateral temporal lift which will alleviate the droopiness of the brow.  We will also carry out follicle unit hair grafting technique to cover any scars which are noticeable as a result of the skin excision.

This procedure can be combined with Erbium laser resurfacing, micro fat grafting and anterior and posterior cervicoplasty which will improve the contour along the angle of the jaw.

This mini facelift technique is unique where we hide the scars with our follicle unit hair grafting technique, augment the malar prominence with micro fat grafting and if necessary improve the jaw contour with cervicoplasty.  All these things are done with minimal down time and reduced complication as opposed to the other extensive procedures.
